Intake questionnaire

IMPORTANT: We limit our practice to cases of termination, failure to promote, demotion, failure to hire, harassment and retaliation. Due to the volume of inquiries we receive, we are unable to provide free advice on matters not involving these employment actions. 

We do offer free consultations. However, this is only offered on a limited basis and only after we have received your intake submission and determined you may potentially have a viable lawsuit. 

DIRECTIONS TO SUCCESSFULLY SUBMIT

The case evaluation is two pages. At the bottom of the first page, you must click the “NEXT” button. At the bottom of the second page, you must click the “SUBMIT” button. After the “SUBMIT” button on the second page is clicked, you will receive a message that the submission has been successfully submitted. You will also receive an email confirmation.

The following questionnaire elicits the information required for us to determine whether your claim meets the legal requirements for discrimination, harassment, retaliation, sexual harassment, failure to accommodate disability, and FMLA interference claims. The case evaluation is free of charge. 

When a prospective client contacts us, we find that they may have more than one claim without realizing it, or have a different claim than they had thought. Although we often follow up, your initial evaluation is limited to the information you provide us. Therefore, it is particularly important that you thoroughly answer the questions to the best of your ability. 

The form will populate questions based on your response to the first question, “What employment issues are you contacting us about?” In addition, other questions may populate depending on your answer to certain questions, and therefore it is important for you to answer each question. 

You may check multiple boxes. For instance, if an employee is subjected to racial harassment, the employer may be creating a hostile work environment. However, the employer may have also denied the employee a promotion due to racial discrimination, which may give rise to a separate discrimination claim. If the employee then files a complaint alleging discrimination in the denial of the promotion, and the employer takes retaliatory action because of the complaint, there may be an additional third claim for retaliation. In this case, you should check the discrimination, harassment, and retaliation boxes and complete the applicable questions.

We also find that prospective clients do not always know what information is relevant and therefore important to provide. Although the questions are designed to elicit specific information, we recommend you review our Know Your Rights pages before completing the questionnaire. These pages answer frequently asked questions and provide a detailed explanation of the legal requirements for each claim, and therefore can provide you with a good idea of what, if any, additional information you should include.

However, whether your claim meets the legal requirements for a claim is not the only consideration when deciding whether bringing a lawsuit would be beneficial to you. In addition, the amount of recovery we may be able to obtain for you must also justify the time and expense of pursuing the litigation. Therefore, the questionnaire also requests information related to your potential economic and noneconomic damages. The information you provide us is confidential. You can upload any supporting documents you may have at the end of the questionnaire. Once you submit your questionnaire, our attorneys will review the information and get back to you as soon as possible.
